PERM Client Intake Checklist

Use this checklist to gather all necessary information before your PERM consultation. Having these documents ready will help our team assess your case, design the job correctly, and file efficiently.

Employer Documents

  • Company support letter or sponsorship authorization.
  • Federal Employer Identification Number (FEIN) verification / IRS EIN letter.
  • Organizational chart showing the position’s place in the company hierarchy.
  • Financial documents — recent tax returns, annual report, or audited financials.
  • Business description — what the company does, number of employees, years in operation.
  • Description of the work location(s) — office, hybrid, or remote arrangement.

Job Position Details

  • Proposed job title
  • Detailed job duties — what the employee will actually do day-to-day (this is the most important item)
  • Minimum education requirement — degree level, field of study
  • Minimum experience requirement — years and type of experience
  • Special skills, software, or certifications required
  • Work location(s) and any remote/hybrid arrangement
  • Proposed salary or salary range

Beneficiary (Employee) Documents

  • Current resume with full employment history
  • Copies of all academic degrees and transcripts
  • Experience / employment verification letters from prior employers
  • Current immigration status documentation (visa, I-94, EAD, etc.)
  • Any prior PERM filings or immigration case history

Recruitment File (Prepared After PWD Approval)

The RIA introduced significant ongoing compliance obligations for Regional Centers, including:

  • Copies of all newspaper advertisements with publication dates
  • State Workforce Agency (SWA) job order confirmation
  • Internal posting notice with start and end dates
  • Documentation for all three additional required recruitment steps
  • Complete log of all applicants with contact information and notes
  • Recruitment report with documented lawful rejection reasons

Important Reminder

PERM audit records must be retained for five (5) years from the date of filing or from the date of certification, whichever is later. Our firm maintains a complete audit file for every PERM case we handle.
